Not all “hormones” are created equal By Tana Richter, BSN, MPAS, PA-C The foundation of ideal women's health begins with good nutrition, healthy lifestyle choices: nutrition, exercise, stress management, enough sleep, and when appropriate nutritional supplements. Once a woman enters menopause the replacement of hormones is necessary to restore balance to the body as well. When it comes times to take hormones it is been found to replace the hormone missing the same bio-identical hormone and to give it a physiological levels is the safest. I have known for years that bio-identical hormones, such as estradiol, estriol and progesterone are definitely safer than synthetic and animal-derived counterparts, such as premarin, provera, medroxyprogesterone or progestins, and prempro. Now we finally have published data to back this claim medically. I have summarized these findings which were recently published in the Postgraduate Medicine journal in January 2009. The data and clinical outcomes showed that bioidentical hormones hands down are associated with much lower risks of the following: breast cancer, cardiovascular disease, and venous coagulation. Until evidence is found to prove otherwise, bio-identical hormones remain the preferred method of hormone replacement therapy (HRT). Proper hormonal balance is critical to your overall health. Hormones are the most finely tuned system in your body. The literature on hormones is vast; we are bombarded with daily information from the television to the internet. Unfortunately, some of the information provided is not only inconsistent but at times also misleading, especially with respect to hormone therapy. In many articles claiming that hormone therapy is harmful the study is referring to the drug or drugs premarin, provera or prempro, which are not bio-identical hormones at all. In fact, I don’t consider these drugs natural hormones at all. This can be very confusing for the lay person. I hope to clear up some of this confusion and make you more aware of how different bio-identical hormones really are compared to their synthetic counterparts. The article reviewed studies that compared differences in breast tissue of those on progesterone versus those on other progestins. Most of the literature showed physiological differences between progestins and progesterone and their effects on breast tissue. Synthetic progestins have potential antiapoptotic effects and may significantly increase breast cancer. On the contrary, progesterone inhibits breast cell proliferation and induces breast cancer cell apoptosis, therefore diminishing breast cell mitotic activity, and stops human breast cancer cells by up regulating cyclin-dependent kinase inhibitors and downregualtion cyclin D1. Synthetic progestins can contribute to a bad estrogen metabolism ratio by promoting the formation of 16-hydroxyestrone. Progesterone has an opposite effect causing 17-beta-hydroxysteroid dehydrogenase, which increases the intracellular conversion of strong estrogens to their less potent counterparts. Synthetic progestins have potentially carcinogenic or cancer causing effects on the body, while progesterone has more anticarcinogenic or cancer protective properties. I have been prescribing bio-identical hormones long before they became popular. The use of bioidentical hormone replacement therapy (HRT) has become very controversial and for the average lay person I’m sure is very confusing. I hope this article helps clear up this confusion. It concludes that bioidentical hormones are associated with much lower risks than synthetic premarin and provera or medroxyprogesterone. In fact, I don’t consider these drugs natural hormones at all. Bio-identical are not drugs. They are natural substances that are vital to your longevity and health.
